FAQ
Welcome to our FAQ section! Here you can find answers to commonly asked questions about our services, appointments, and what to expect during your visit. If you have any other questions, feel free to reach out to us directly.
Bespoke Physical Therapy means your care is completely tailored to you. We design personalized treatment plans based on your unique goals, lifestyle, and body. Rather than following a standard program, our therapists take the time to understand your movement patterns, medical history, and daily demands to create a plan that fits you perfectly.
Whether you’re recovering from an injury, managing pain, or looking to improve performance, our bespoke approach ensures that every session is focused on helping you move better, feel stronger, and live without limits.
Currently in Missouri the law allows patients to seek physical therapy care without a physician referral. Removing this requirement will significantly reduce costly delays in accessing high quality care.
If you are seeking care for a pain problem, we are still required to communicate with your other health care provider (Primary Care Physician, Physician Specialist, Chiropractor, Dentist, etc). For this reason, we will be asking for their name and contact information, if you have it.
A key component of our education as doctors of physical therapy is to thoroughly screen your medical concern and know when your medical concern is not in our scope of practice. If this is the case, or if you do not make improvement within a specified time frame, we are required to refer you to an appropriate healthcare provider.
If your referring provider wrote a prescription for physical therapy, please bring or send a copy for us to review. Your physician can also fax it directly to us at 314-.
If you have any questions about referrals, do not hesitate to contact us at info@bespokeptstl.com
The short answer is no, we are a cash based out of network provider.
After years working in traditional outpatient clinics, Bespoke PT founders Taylor Bushnell and Zach Boyle saw firsthand how insurance-driven care models limited the time, attention, and resources that patients truly need to achieve long-term movement health.
These limitations often looked like:
Double or triple booking patients
Relying heavily on assistants instead of direct PT care
Rushed appointments with little time for questions
Insurance requiring 2–3 visits per week—or forcing discharge before goals were reached
Taylor and Zach believe patients deserve better. That is why they started Bespoke Physical Therapy, a clinic built on quality, expertise, and individualized care—not insurance restrictions.

Why We Are Out-of-Network
To deliver this level of care, we operate out-of-network with insurance companies. This allows us to:
Control session length
Avoid high patient volumes and rushed visits
Provide individualized, evidence-based treatment
Keep your care between you and your PT, not dictated by insurance policies
To Check on your Out-of-Network coverage please go to Reimbursify.com
While we do not accept direct payment from insurance, you can still submit for reimbursement after your visits if you ask for a superbill at the time of service.
Medicare Patient Information
We do accept Medicare part B, otherwise please see the FAQ for other medicare plans.
How to Submit a Superbill for Insurance Reimbursement
How to submit a superbill in Reimbursify
1. Log in to Reimbursify
Open the Reimbursify app or go to https://reimbursify.com
2. Start a new claim
Tap or click “Submit a Claim”
Choose Physical Therapy
3. Upload your superbill
You can upload the superbill by:
Taking a photo
Uploading a PDF
Selecting a saved file
Reimbursify will automatically scan the document and extract information.
4. Review and confirm details
Carefully check:
Provider information
CPT & ICD-10 codes
Date(s) of service
Amount paid
Fix anything that looks incorrect—this step is important to avoid denials.
5. Add insurance information
If not already saved:
Select your insurance provider
Confirm your member ID
Choose out-of-network (superbills are almost always out-of-network claims)
6. Submit the claim
Tap Submit
Reimbursify will either:
Submit electronically to your insurer, or
Generate a ready-to-mail claim (they’ll guide you if mailing is required)
After submission
Track claim status in the app (e.g., Submitted → Processing → Paid/Denied)
If approved, reimbursement is sent directly by your insurance company, not Reimbursify
Typical processing time: 2–6 weeks, depending on your insurer
Common reasons claims get denied (and how to avoid them)
❌ Missing diagnosis codes → Ask your provider to include ICD-10 codes
❌ Provider NPI missing → Required for most insurers
❌ Wrong insurance selected → Double-check the plan
❌ Illegible photo → Use clear, well-lit images or PDFs
If you are Medicare can you be seen at Bespoke?
Yes, however we are only contracted with Medicare part B plans. If you have medicare part B we can accept your insurance and treat you at Bespoke.
If you have Medicare Advantage the answer is more tricky. Bespoke is out of network with all commercial insurances. Some advantage plans allow you to self pay for services, but your specific plan must specify it. We partner with Reimbursify and through their services you can check and see if you are eligible to be treated at Bespoke. You must also confirm that prior authorization is not required.
What Dry Needling Is
Dry needling uses filiform needles to:
Release or deactivate trigger points
Reduce muscle tightness
Improve range of motion
Decrease pain
Stimulate healing by increasing blood flow
It’s called “dry” because nothing is injected through the needle.
How It Works
A provider inserts the needle directly into a trigger point—a knotted, hypersensitive area within a muscle. This may cause:
A quick twitch response
A feeling of pressure
Temporary muscle soreness afterward
The twitch response is believed to help the muscle reset and relax.
What It’s Commonly Used For
Dry needling is often used to treat:
Muscle tension and spasms
Sports injuries
Neck and back pain
Shoulder or hip tightness
Tendon issues
Headaches related to muscle tension
Many patients feel immediate or gradual improvement.
How It Differs from Acupuncture
Although the needles look similar, dry needling is:
Based on Western medical principles (anatomy, physiology)
Focused on musculoskeletal dysfunction
Applied specifically to trigger points
Acupuncture is based on Traditional Chinese Medicine theory and targets “meridians” or energy pathways.
Is It Safe?
When performed by a trained, licensed provider, dry needling is considered safe. Possible temporary side effects include:
Mild soreness
Bruising
Fatigue
Rarely, more serious complications like pneumothorax (extremely uncommon)
What Manual Therapy Is
Manual therapy refers to any therapeutic technique a clinician performs with their hands, including:
Joint mobilization
Joint manipulation (high-velocity, low-amplitude thrusts—depends on provider scope)
Soft tissue mobilization
Myofascial release
Trigger point pressure
Muscle stretching
Scar tissue mobilization
Lymphatic drainage
The goal is to restore movement, reduce pain, and improve tissue health.
What Manual Therapy Does
Manual therapy can:
Decrease muscle tightness
Improve joint mobility
Reduce pain
Increase circulation
Improve alignment and posture
Help tissue healing
Reduce swelling
It’s often combined with exercise therapy to produce longer-lasting results.
Conditions Manual Therapy Helps With
Manual therapy is commonly used to treat:
Neck and back pain
Joint stiffness (spine, shoulder, knee, hip, ankle, etc.)
Muscle tension
Headaches
Sports injuries
Postoperative stiffness
Tendon and ligament injuries
Temporomandibular joint (TMJ) dysfunction
What a Typical Session Looks Like
During a session, your provider may:
Assess the painful or restricted area.
Use hands-on techniques—gentle or firm pressure, stretching, joint glides, or mobilizations.
Combine manual therapy with therapeutic exercises before or after.
Sessions are tailored to the patient’s needs and comfort level.
